I am applying for the following class of membership (check one):

___ FULL MEMBERSHIP‑ This includes all club privileges and is open to licensed amateurs in good standing with the FCC. Full members shall have full rights to vote on all club matters and to hold a club office. Membership is based on meeting the following requirements.

A. Payment of annual dues in the amount(s) required.

B Written expression of willingness to abide by the Articles of Incorporation.

by‑laws, and Federal Communications Rules and Regulations pertaining to the Amateur Radio Service.

C. Submission of a photocopy of the prospective member's Amateur Radio License.

D. Acceptance by membership committee appointed by the Board of Directors.

E. Approved by the membership.

___ ASSOCIATE MEMBERSHIP‑ Any unlicensed amateur operator who has applied and been approved by the club for membership. Associate members have all the club privileges of a full member except for the right to vote and to hold an elected office.

___ FAMILY MEMBERSHIP ‑ This membership is open to all licensed or unlicensed family members of other persons (18 years of age or younger) living in the same household as a full member. Only licensed amateur radio operators have the same privileges as a Full member. Family members, licensed or not, have the same privileges as Associate members. Licensed Family members may become Full members by paying $15.

Annual Dues for FULL and ASSOCIATE members are $15. FAMILY members dues are $3 annually. You may pay by cash, check or money order.
