In order for this service to be utilized, the customer must either download our form online (available at http://www.bayou.com/docs/ACH.pdf) and print it, come into our location at 1109 Hudson Lane, Monroe, LA or contact our billing department at 318-323-0011 or 1-888-302-2968 EXT 2035 and we will mail you a form.
This form cannot be emailed, faxed or received in any electronic form. We must receive the original signed form. It must be filled out completely in order to be valid. We must have a copy of a voided check along with the form.
All accounts are drafted on the 5th of every month.
